Well, you checked with your medical providers, so IMO you're doing all the right things. Yeah, 391 is pretty high. Too high to exercise much, so short of eating way low carb, all you can do is just keep on with the insulin and the testing, and just do what the doc's office told you to do.

Yes, with a new insulin user, they have to start with a small dose and then systematically raise it. This slow steady way of doing things takes a lot of patience when you're anxious to see better numbers and to feel better, but it really is the correct way to do it. Not sure why she said 20-35 units will do it. A lot depends on his weight, activity level, how much insulin resistance he has, what he eats. Lots of us take way more than 35 units of Lantus every day.

Make it as close to every 24 hours as you can without being a slave to the clock. With Lantus, you have an hour or so leeway to play with if you have to. But if there's no reason to make the time different, go ahead and nail the 24 hour mark as close as you can. Have him take it at supper if his bedtimes are irregular. As long as whatever he does is roughly the same every day, that'd be fine. This is going to be a long trek, Donna, to get your DH where he needs to be. Try not to worry too much.
