I know someone who low-carbs who probably eats 5k calories a day and is perpetually hungry. She lost about 70 pounds, but (not unsurprisingly due to the huge calorie load) can't lose more. I have the feeling she is one of those people from HOWL who report being able to eat until they vomit and still feel hungry. It is apparently something in the brain satiety mechanism gone awry. It could be that this individual also had a misfire in the brain, or it could be (as you speculated) that he could not consume enough to be satiated. I've known a person or two who got nauseated from a moderate fat diet.
The doctors at Tufts were thrilled with all my tests (other than the A1c at 6) because all of them were right in the middle of normal. Low carb does wonders for all the pesky triglicerides, LDL/HDL, etc. Then again, the bariatric docs there think low carb is the only viable alternative.
